Event Request Form

Full Event Support — (suggested budget of > $65pp if serving food/beverage) Our team will provide support from planning to event execution, including: sourcing & coordinator with vendors (event space, rentals, catering, A/V, florals, photography, parking/security); create & manage event in DAD; draft & send email &/or print invitations, create and send pre- and post-event emails; print name tags, place cards, &/or menu cards; provide host gift & event decor; provide on-site event support. Note: we can be the best partner with at least 60 day’s notice.

Partial Event Support — On-site event support will be evaluated on a case-by-case basis, but we are happy to provide consulting services, such as: working with you to source and book event space and rentals; providing a checklist of important event tasks to consider in your planning; printing name tags; providing host gift and event decor.

Event-In-A-Box Support — We are happy to provide any event supplies you may need, including: name tags (pre-printed or blank/sticky), a host gift, guest “swag” or event decor.
